We are dedicated to the care and treatment of patients with kidney disorders, kidney transplant patients and patient who are on dialysis. We have multiple locations servicing greater Columbus area for your convenience.

Please note: If you need to change the date or time of your appointment, please call at least 24 hours before your scheduled appointment time to avoid a late cancellation fee. Please arrive 15 minutes prior to your appointment time.
Please bring the following items with you to all of your appointments:
- Referral from your primary care physician (if applicable)
- Insurance Card
- Any copay amount you are responsible for (if applicable)
- A detailed list of all medications you are currently taking including dosages or medication bottles
Office Hours and After Hour Contacts
Our office hours are Monday through Friday from 8:00am to 5:00pm except holidays. After hours, our on-call physician can be reached by calling (614) 839-1541.
In the case of a medical emergency, please call 911.
Insurance Plans and Billing
Columbus Kidney Care accepts most insurance plans in our area including Medicare, Medicaid, HMO's, PPO's and indemnity insurance plans. Please check with your insurance company to verify that we are providers for your plan. If you are part of an HMO or other plan that requires referrals to our office, please coordinate with your primary care physician for a referral prior to your appointment with us. If we are not participating with your plan, please let us know, and we will work with you to obtain an individual agreement for your care.
Our billing staff works together with our patients to ensure services received are processed correctly and expediently. The billing office may be contacted at (614) 839-0581 Option.2 regarding your concerns.
Pharmacy and Prescription Refills
If you need a prescription refill, please contact your pharmacy. The pharmacy will send us a fax or electronic request for your refill. Please allow 48 hours for a routine refill. If you have a prescription related question, please call our office during normal business hours, Monday through Friday, 8:00am to 5:00pm at (614) 839-0581 Option 1.
